This policy is for Washington residents and anyone whose health data we collect in Washington, under Washington's My Health My Data Act. It is from Pure Vanity Wellness LLC, doing business as Olena's Spa and Laser, and adds to our Privacy Policy.
Your answers to our health questions (conditions, pregnancy, medicines, allergies, skin conditions, hormone conditions), skin and hair type, treatment notes and results, treatment photos, and the fact that you asked about or booked a treatment.
Directly from you (forms, consults, messages), from our own observations and photos during treatment, and from our booking system when you book.
To decide whether a treatment is safe for you, to provide it, to track your results and the Smooth Results Promise, to contact you about your appointments, and to keep the records the law requires. We collect only what we need to provide the service you ask for. We use it for anything else only with your consent.
Our service providers that store it or help us provide your care (Aesthetic Record for client records and HighLevel/LeadConnector for booking messages), a medical provider if you agree to a referral, and authorities when the law requires it. We share it with anyone else only with your separate consent. We never sell health data.
You can ask to confirm whether we have your health data and see it, get a list of everyone we shared it with, withdraw your consent, and have it deleted. Email [email protected] or call (503) 775-0900. We answer within 45 days, free of charge. If we turn down your request, you can appeal by replying to our answer, and we respond within 45 days. If you disagree with the appeal result, you can contact the Washington State Attorney General.
